The following paperwork must be completed and handed in to the instructor the first day of class:

1.  Segment 1 Contract - This is available online or can be obtained by visiting Portland High School's main office. The contract must be signed by both the parent and the student.

2.  Segment 1 Registration Form- Please print, complete the information.  This information will be used to contact the student or parent in case of any changes in schedule.  The parent waiver is a voluntary sheet.  The state requires that there are two students in the vehicle with the instructor at all times unless the parent wavier sheet is signed.  If a student cancels a ride, and you have not signed that sheet, I will not be able to take your student on their drive, and it will have to be cancelled. 

3. Copy of Birth Certificate - I must see a copy of the birth certificate to confirm the students age at least 14 years 8 months on the first day of the Segment 1 class.  The copy will be returned to you with the copy of your contract.

4. Payment due first day of class.
